2020-2021 Electronic Forms
To save you time and a trip to the school, Fairfield Elementary School is working to make many of our required forms available online for completion. You must complete a form for each of your applicable students unless indicated otherwise. Forms submitted online, do not have to be submitted in person.
Families are now able to complete the following forms and submit them back to the school via email:
*If you intend to purchase a protection plan for a Chromebook, you must submit this form in-person at the time of purchase.
TO SUBMIT COMPLETED FORM:
*Your device must be able to read Adobe PDF files in order to complete these forms
1. Click on form to open it in a new window
2. Download the form to your device and open it
3. Complete the entire form (form will not save if required fields are not completed.)
4. Save the form to your device
5. Open your desired email application, and create a new email
6. Attach all documents you will like to submit
7. Send Email to our Parent Liason, Loreal Nelms at firstname.lastname@example.org
